Sometimes the hardest part of a mental health struggle isn't the struggle itself. It's proving it — to an employer, an insurer, or a school — while you're already stretched thin.
As a licensed clinical psychologist in California and Maryland, I'm able to provide clinical documentation to support:
Medical Leave (FMLA-related)
I provide clinical documentation supporting medical leave requests when a mental health condition is affecting your ability to work.
Short & Long Term Disability
I complete clinician sections of disability paperwork and provide supporting letters based on your diagnosis, symptoms, and functional limitations.
Workplace Accommodations (ADA-related)
Every project is different. We stay flexible and responsive to make sure the process fits your flow—not the other way around.

A note on scope: I am not able to provide custody evaluations, parenting-time assessments, or any family court–related evaluations. I do not conduct independent medical examinations (IMEs) or forensic evaluations for litigation. I do not make disability determinations — that decision belongs to your employer, insurer, or the relevant agency; I provide clinical information to support that process. And I'm not able to complete this documentation without an established clinical relationship, so a real intake or ongoing treatment always comes first.

In the interest of transparency and informed consent, it's important to be clear about the limits of this service:
This is not a custody evaluation, parenting-time assessment, or any family court–related evaluation. I do not conduct custody evaluations and do not accept referrals for that purpose.
This is not an independent medical examination (IME) or forensic evaluation for litigation. I am not a neutral third-party evaluator; I write from my role as your treating clinician.
This is not a disability determination. The final decision on a disability claim, leave request, or accommodation rests with your employer, insurer, or the relevant agency (e.g., SSA). I provide clinical information to support that process — I don't make the determination myself.
This is not available without an established clinical relationship. I'm not able to write a letter for someone I haven't properly evaluated, no matter how time-sensitive the request feels. If you need something urgently, let's talk about timeline honestly, together.
This isn't paperwork-on-demand. Thoughtful documentation requires that I actually know your clinical picture — your symptoms, how they affect your functioning, and how they connect to the leave or accommodation being requested. Because of that:

If you're new to my practice and are seeking this support specifically, we'll begin with a clinical intake to establish an accurate picture of your symptoms and functional impact before I complete any forms or letters. This protects the integrity of the documentation — and, ultimately, protects you.
Typical documentation may include:
A letter summarizing diagnosis (where appropriate), clinical presentation, and functional limitations
Completion of employer, insurer, or school accommodation forms tied to your treatment
Coordination with a case manager, HR representative, or disability carrier, with your written authorization
A Note on Fees
Clinical documentation — letters, form completion, case manager calls — falls outside a standard therapy session and is typically not billable to insurance. There is a separate fee for this work, based on time and complexity, which we'll discuss before any documentation begins.
